20). WHO MAKES AN ACCIDENT INVESTIGATION REPORT?

21). WHAT IS THE USED OF ACCIDENT REPORT? 
To find out the root cause of accident makes recommendations to prevent re-occurrence and evaluate the effectiveness of emergency response.
